Requests for old paper medical charts (private practice records prior to April 10, 2012)
Our medical practice utilized paper medical charts until April 10, 2012. The retention requirement for adult medical records is seven (7) years. Accordingly, all paper medical and billing records from our practice have been destroyed by Cor-O-Van, the medical records storage company utilized by our office.
Requests for private practice electronic records (April 10, 2012 - December 30, 2014)
Our private practice used the athenahealth electronic health record (EHR) from April 10, 2012 through December 30, 2014. If you had an office visit during those dates, a medical record may exist. Upon our affiliation with Sutter Pacific Medical Foundation (SPMF) on January 5, 2015, all of our existing patients were required to sign a transfer of care from our private practice to SPMF. Upon receipt of the patient's signed authorization form, the prior athenahealth EHR was transferred to Sutter Health as a PDF file and was uploaded to the "Media Tab" of the patient's Sutter Health EHR, created in early 2015.
Requests for medical records from this period of our private practice (April 10, 2012 - December 30, 2014) must be made in writing by way of a signed medical records authorization release form to Sutter Health. This form is available by contacting any Sutter Health care center, including our former SPMF care center at 45 Castro Street, Suite 402, San Francisco, California 94114. The phone number for our former care 45 Castro Medical Office Building care center is (415) 600-4750. Sutter Health is responsible for providing a copy of your electronic medical record (EHR) from this period to you and/or your designee (new health care provider, clinic, etc.)

This form is available by contacting any Sutter Health care center, including our former SPMF care center at 45 Castro Street, Suite 402, San Francisco, California 94114.
